Description Community Living Chatham-Kent  
Two separate locations that provide support during the day to promote skill development and enhance quality of life while providing respite for the families of diagnosed intellectually disabled individuals * Provide activities to enhance the quality of life and the development of life skills. Riverview Day Program: 650 Riverview Drive Chatham * Program includes Life Skills; Fitness and recreation; Computer lab; Industrial assemblies and sub-assemblies for local businesses; Cafeteria area prepares food for lunches and breaks; extended program on Friday evenings for people who reside with families. Sass Road Day Program, 130 Sass Road, Chatham * Program includes: Personal care and support; Enhancement of quality of life; Life Skills; Support and guidance to participate in activities of daily living; Extended Day Program available five days per week for people who reside with families, provides personal care to assist families. Day Programs staffed with professional support workers employed by Community Living; Parents are not required to attend with their child * Most individuals in the programs consider it to be going to work * Families provide their own transportation or subsidized transportation is offered at a cost of $100/mth for door to door service.* Community Living Chatham-Kent believes in working with the individual and the family to meet their needs

ApplicationAll adult persons who require service must apply through Developmental Services Ontario, South West Region, 1-855-437-6797. Once the person has been assessed by the DSO, referral will be made to Community Living Chatham-Kent when a space becomes available.
