|Description||The Erie St. Clair CCAC provides access to in-home health and personal support services to help individuals live independently at home. We also assist community members with the transition to long-term care homes when living at home is no longer possible. The Erie St. Clair CCAC can provide information about, and can link individuals to, other helpful services available in the community. Case Managers are health care professional specially trained to help clients investigate and organize their health care services so that clients are able to achieve the best possible state of health and well being. |
The Erie St. Clair Community Care Access Centre (CCAC) serves the Chatham-Kent, Sarnia-Lambton and Windsor-Essex communities.
Your Case Manager will:
Take the referral from your call (or a call on your behalf)
Answer your questions; Determine your eligibility for services (according to Ontario Ministry of Health and Long-Term Care Guidelines; Assess your needs by talking with you and your caregivers; Provide health education and information
Create goals and a specialized care plan with you and your caregivers; Arrange for you to receive the appropriate services for your situation (e.g., nursing, personal support, and/or any therapies); Liaise with your service providers and other members of your health care team; Be a resource for you and your caregivers; Reassess your needs regularly; Help you create a plan for when your goals have been achieved; Link you with other community agencies and services; Help you find alternate sources of care if necessary.
Speciality programs are available:
Falls Prevention, Palliative Care (In-home hospice); Geriatric Assessments; Acquired Brain Injury; Elder Abuse Prevention CCAC Services: Case Management; Information and referral; nursing; personal support and homemaking; respite care; eligibility assessment for admission to long-term care homes and senior day programs; occupational therapy; physiotherapy; social work; speech language pathology; dietetic counselling Charitable donations to CCAC are used to purchase equipment for loan to families such as comfort mattresses and to support direct service education for staff
Information and Referral:
If you are not eligible for CCAC services, or if we are unable to provide services to meet all of your needs, we will help link you to other services available in your community.
Professional staff will help you find the right service for your particular needs. These may include:
Adult day programs; Meal delivery services; Assistance with shopping, cleaning or home maintenance; Transportation assistance .
Anyone can make a referral to the CCAC – you can refer yourself, or have a family member, friend, caregiver, physician, or other health care professional make a referral.

|Eligibility||Infants; Toddlers; Children; Youth; Adults; Seniors; Male; Female; Ontario Health Insurance Coverage required; Service is available for all age groups, from newborns to elderly residents of Erie St. Clair. Needs are identified and services determined according to Ministry criteria. Eligibility and service needs are reassessed on a regular basis.|
