| Description | CHAP Seniors Transportation (fares depend on location within Chatham-Kent, can be as low as $7.50 round trip). Service to many locations throughout Chatham-Kent and surrounding area. Client is accompanied by a caring, dedicated volunteer or staff person. Wheelchair accessibility can be accommodated. Nutritious Frozen Meals (Over 100 items on menu. Entrees $4.25, Soups, desserts and breakfast items $1.50). Nutritionally balanced, home-stykle meals prepared by dietician specifically for seniors/diabetics. Meals are easily cooked in oven or microwave. Specialized diets are available. Free delivery to those unable to pick up. Home Helper $14.00/hr min 2 hrs charge for duties inside the home including companionship and personal care, relief for family members or caregivers. Promotes home safety and quality of life. Housekeeping, laudry, cooking available. Home Maintenance such as grass cutting, yard maintenance and snow removal $15.00 hr. Caregiver Counselling (free of charge). Client Intervention & Assistance (free of charge).Transitional care planning, elder abuse prevention, accessing services and community referrals.
