| Description | Provides mental health support to adults with a serious mental illness Assists individuals to maintain a good quality of life while living independently in their community * works with individuals to maintain their residence and navigate social services such as Ontario Works and Ontario Disability Support Program * helps develop a supportive social network, manage daily living skills such as budgeting, shopping, social skills, and develop new and effective coping strategies * provides access to a supportive advocate in times of difficulty or crisis | |||
