| Description | Provide eligibility assessment, coordination of care, placement services and system navigation for those in need of community-based health service Assessment for in-home services and placement also provided * in-home services include nursing (either at home or in flex clinic settings), nutrition services, occupational therapy, physiotherapy, social work, speech-language pathology, personal support and homemaking services * placement services can include placements into Long-Term Care Homes for both long-term or short-term and adult day programs, rehabilitation and complex continuing care beds * access to assisted living and supportive housing | |||
