| Description | Offers a wide range of health care services to individuals in their own homes Advanced Home Care Team - A team of Nurse Practitioners who work closely with family physicians, care coordinators and other health professionals Nursing Services at Home - Shift and Visiting - Provides nursing services in the home for individuals who are home-bound Nutrition Services - Provides nutrition counselling services for individuals in their home Occupational Therapy - Provides occupational therapy services for individuals in their home Personal Support Services - Personal support services are provided to individuals who requires assistance with care needs, including personal hygiene and activities of daily living Physiotherapy - Provides physiotherapy services for individuals in their home Social Work - Provides social work services for individuals in their home Speech-Language Pathology - Provides speech-language pathology services for individuals in their homes for swallowing and/or speech problems | |||
