DescriptionProvides access to community care * links people to a wide range of community services and supports * care coordinators work with clients in community and hospital settings to determine what help they need and arrange for care * care is delivered to the client in various community settings including home, school, residential facility and at flex clinics

Services available to eligible clients may include:
  • Personal support
  • Nursing
  • Nurse practitioner
  • Physiotherapy and occupational therapy
  • Speech-Language therapy
  • Social work
  • Nutritional Counselling/Dietetics
  • Medical supplies and equipment
  • Connecting to other community support services and agencies

Other specialized services may include:
  • Access to long-term care homes, rehabilitation and complex continuing care beds
  • School health support services to school aged children
  • End-of life care
  • Access to respite care and convalescent care (short-term stays and adult day programs)
  • Community support for people living with cancer


Care Coordination - Provide eligibility assessment, coordination of care, placement services and system navigation for those in need of community-based health service

Geriatric Resource Team - Specialized team of Care Coordinators trained to assess the special needs of the elderly

In-Home Services - Offers a wide range of health care services to individuals in their own homes

Information and Referral - Connects individuals with the right community services and supports to assist in management of health care needs

Nursing Services in Flex Clinics - Provides nursing services for ambulatory clients at clinics located in the community

Respite - Respite care services are provided when an individual cannot be left alone or is at significant risk if left alone and the primary caregiver requires relief from direct care

School Health Support Services - Provides special health services at school which are essential to a student's participation and development in school programs

Supportive Care Resource Team - Specialized team of Care Coordinators trained to support people who are living with, or dying from a progressive, life-limiting illness and those with a cancer diagnosis
