DescriptionHealth centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services


Assisted Living Program - Support staff provide personal care, medication assistance, light housekeeping, security checks and meal planning

Chiropody Services - Treats a wide variety of foot problems - including bio-medical assessments and provides foot and ankle orthotics * specializes in foot ulcers

Clinical Dietitian Services - Nutritional information and education to people requiring assistance * diabetes education and support teams on staff * one-on-one, couple or family sessions available for people with food and health related concerns

Community Engagement - One day community events to engage and build community capacity

Community Support Services for Seniors - Helps maintain independence while the person remains in their home and community with various programs

Cooking Classes - Cooking lessons

East Elgin Diabetes Team - Supports for people living with prediabetes or diabetes * one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff

Health Promotion and System Navigation Services - Provides one-on-one and group sessions to promote health and to assist people in navigating the health system connecting with community programs and linking to available social services * group programming and one-on-one counselling focused on improving community health

Little Learners Tutoring - One on one weekly tutoring sessions for children entering JK, SK and Grade 1 *

Mental Health Services - Confidential counselling for all ages

Parenting Support - Parenting support and education classes offered on a rolling schedule

Primary Health Care Services - Provide primary health care services to community
