| Description | Offers health and social services to people who are homeless as well as those at risk of homelessness Interdisciplinary care team operates within a health promotion framework and values inclusivity, relationship building, and harm reduction * services include nursing care, addiction assessments, screenings, referrals to treatment, harm reduction education and support, counselling, advocacy, outreach, crisis intervention and referrals for foot care and diabetes care Health outreach also offers the following: | |||
