Community Support Services - Physical and Developmental Disabilities - Daily living skills and community training

Personal Support/Independence Training - Provides educational, community and recreational training to people with physical and mental disabilities

Project Hope - Full-day program designed to meet the individual needs of young adults with developmental and/or physical disabilities who live at home with their families

Supportive Housing - Shared living options (3-4 people) for individuals with physical and multiple disabilities

Supportive Housing - Small - Small shared living options (2 people) for individuals with physical and multiple disabilities and those with acquired brain injuries * accredited with FOCUS
HoursAdministration: Mon-Fri 9am-5pm
EligibilityAges: 18 year(s) and up 
 
Physically and intellectually challenged individuals * primary diagnosis must be acquired brain injury (ABI)
ApplicationDevelopment Services Ontario (DSO) agency referral * self and family referral
