DescriptionCoordinated Intake is the single point of entry for all community referrals to access adult outpatient programs

Coordinated effort between London Health Sciences Centre (LHSC) and the Ambulatory Services attached to the Mood and Anxiety and Psychosis Program at Regional Mental Health Care–London  Regional Mental Health Care-London (RMHC-L)
HoursMon-Fri 8:30am-4:30pm
EligibilityAges: 16 year(s) - 65 year(s) 
 
Adults and youth 16 years and older who are experiencing a mental health crisis * current/pending criminal charges are exclusion for group therapies * Eligibility requirements include:
  • being between the ages of 16 and 65
  • being a resident of London and Middlesex County
  • meeting the admission criteria of the service
  • having neither pending criminal charges nor active substance abuse use
ApplicationA physician referral is required * family doctor or a walk-in clinic may make the referral * doctors are also encouraged to send any available reports or information relevant to the referral
