DescriptionThe Community Health Centre offers primary care services for the health needs of individuals and families * The centre operates under the Collaborative care model (a process of collaboration between family doctors and the shared care clinical team of mental health professionals)

The shared care clinical team offers care using best practice guidelines and referral processes that are in place to a wide network of specialists and to other education and support programs for chronic disease management such as; Family practitioners, Nurse practitioners, RPN, Nutrition, Therapy and Health promotion programs

EligibilityAges: 16 year(s) - 64 year(s) 
 
Individuals with no family doctor and live within the catchment area bounded by Crawford Ave, Tecumseh Rd, Central Ave and Riverside Drive and/or have a diagnosis of severe mental illness living anywhere in Windsor and Essex County.
ApplicationClinic currently not accepting new clients; 
Referral considered on a limited basis and available services
