Description (Service) | COVID-19 - Transportation Services has resumed regular operations and grocery runs are adjusted to accommodate senior shopping hours. The number of passengers per vehicle is limited to allow for physical distancing. Payments are requested via e-transfer, VISA, or invoicing to reduce handling cash. Disinfecting, PPE, and screening protocols remain in place. Frozen Meals can be delivered free within Chatham-Kent (front door/porch drop-off) twice weekly and/or picked up curbside at the agency. Payments are requested via e-transfer or VISA, with invoicing available if required. Please phone in advance to make arrangements for meals. Grocery Runs: seniors and persons with disabilities will be driven in an accessible vehicle to and from the grocery store to shop during dedicated senior/vulnerable persons hours. One are of C-K is served per day and passengers must phone at least 3 business days in advance to book a seat the vehicle. Clients must provide an exact list of items requested to their matched volunteer, along with cash (in a Ziploc) or debit (tap only, we will not take client pin numbers). Volunteers will shop at the requested grocery store during dedicated Senior/Vulnerable Persons hours when possible. Client Intervention & Assistance services are provided in the community by a registered social worker who provides urgent intervention and support in critical situations. Services are available for vulnerable and at-risk seniors, persons with disabilities, and residents of care homes and care facilities. Services are client directed, confidential, and delivered at no cost to the client. Our CIA worker is available to provide service coordination over the phone or internet in the event that an eligible individual requires assistance accessing food, emergency food resources, and/or information. Offers the following services to seniors and adults with disabilities: CareLink Health Transit - Service to many locations throughout Chatham-Kent and surrounding area. Client is accompanied by a caring, dedicated volunteer or staff person. Wheelchair accessibility can be arranged. Frozen Meals - Nutritionally balanced, home-style meals prepared by dietitian specifically for seniors/diabetics. Meals are easily cooked in oven or microwave. Specialized diets are available. Delivery available. Home Help - Duties inside the home including: companionship and personal care, relief for family members or caregivers. Promotes home safety and quality of life. Housekeeping, laundry, cooking available. Home Maintenance - Grass cutting, yard maintenance and snow removal. Client Intervention & Assistance - Transitional care planning, elder abuse prevention, accessing services and community referrals. Dealing with concerns that affect safety and well being. |
