| Description (Service) | Offers primary health care for residents in Tilbury and the surrounding area. An inter-professional group of health care practitioners, including family physicians, provide health care in an integrated, respectful and holistic environment to help keep patients healthy before illness occurs. The focus of the health team is on disease management and prevention, acute care and health promotion. Team consists of: family physicians * nurse practitioners * dietitian * social workers * pharmacists * chiropodist * registered nurses * diabetes and lung health educators * phlebotomist * respiratory therapist Advisory services include: * Assisting patients navigate through the heath care system * Providing primary health care, chronic disease management and self-help tools to improve health Also partners with numerous other agencies to bring their services and expertise to our community. |
