Dedicated to enhancing the health, quality of life and independence of individuals in our communities by offering a single point of access to home and community services. Our services include: case management, specialized nursing programs, nursing clinics, personal support services, occupational therapy, physiotherapy, social work, speech-language pathology, information and referral to community health and social services, services for children and youth, palliative care consultation, telehomecare and much more. |
