| Description (Service) | End-of-life social, psychological, cultural, emotional, spiritual and practical supports * care plans are customized to the client and family's goals, wishes and priorities * support, education and consultation to community service providers, including basic and advance palliative-care training * grief or caregiver support and education * one-on-one and groups for adults and children * phone and social media support, education and resources * complementary therapies and other services also available |

| Meetings | Support services on Monday mornings through the offices of the East Elgin Family Health Team in Aylmer * after-hours individual or group support programs may be provided by pre-arranged schedule * sessions available at locations in West Lorne and Rodney as needed |
