Description and Service Details
Description (Service) Provides client-centered services to people affected by an acquired brain injury (ABI)* services are accessible, evidence-based and responsive to the changing needs of clients, community, and partners to support individuals to maximize their strengths and live healthy, meaningful lives

Focus of rehabilitation is to develop and improve functional skills to increase independence and improve quality of life reflecting best practices* all services are client centered, strengths based, least intrusive, evaluated, and provided in a respectful, ethical manner and wellness, interpersonal skills, and meaningful activity (WIM) goal driven

Gateway to Connections - Adult Day Program for adults living with the effects of an acquired brain injury(ABI)

Assisted Living Services - 24/7 assisted living services in a structured, safe environment within the London community for adults living with an acquired brain injury (ABI)

Consultation and Training - Consultation and training for caregivers, families, service providers and clients by Behaviour Therapists, Clinical Psychologist, and Rehabilitation Counsellors

Cornerstone Clubhouse - Adult day program for adults living with an acquired brain injury (ABI) which offers a place for Members to belong, the opportunity for meaningful work and the opportunity to develop meaningful relationships

Outreach Services - Provides services that are individually tailored and flexible for people living with the effects of an acquired brain injury (ABI) within their own communities
Hours Services: Daily 24 hours

Eligibility Ages: 18 year(s) and up 
Be a Canadian citizen or landed immigrant at least 18 years of age and have an acquired brain injury, including traumatic brain injury, infection, anoxia, aneurysm, tumor and stroke * must have valid Ontario Health Insurance Plan (OHIP) card * must have free will commitment to rehabilitation and/or participation in Dale Brain Injury Services programs * serious medical issues must be stabilized
