Description (Service) Provides client-centered services to people affected by an acquired brain injury (ABI) * services are accessible, evidence-based and responsive to the changing needs of clients, community, and partners to support individuals to maximize their strengths and live healthy, meaningful lives

Focus of rehabilitation is to develop and improve functional skills to increase independence and improve quality of life reflecting best practices * all services are client-centered, strengths-based, least-intrusive, evaluated, and provided in a respectful, ethical manner * wellness, interpersonal skills, and meaningful activity (WIM) are goal-driven

Assisted Living Services - Providing 24/7 residential services in a structured, safe environment within the London community * services are flexible and individualized, with a focus on increasing and maintaining skills and enhancing quality of life * service length ranges from short-term to lifelong

Community Transitional Services - Community Transitional Services delivers one-to-one transitional support, rehabilitation and service coordination to individuals in their home or community * services are flexible and focus on skills training, improving quality of life and making connections with other service providers and community resources * more intensive services are available to support transitions from hospital to home or long term care * services are available across the South West and are offered for up to 12 months

Consultation and Training - Provides general education to care partners, families and service providers about Acquired Brain Injury (including stroke) in addition to offering support and training to assist individuals and teams in supporting and interacting more effectively with clients or loved ones who experience behavioral, social and cognitive challenges after acquired brain injury * services are available across South West

Group Services - Available across the South West, Group Services are flexible and provided in a group setting focusing on individual and group goal achievement, increased independence and quality of life * groups provided offer social, recreational, wellness, skill building, exercise and therapeutic activities * services are available on a short-term, transitional or lifelong basis

Eligibility Ages: 18 year(s) and up 
 
Be a Canadian citizen or landed immigrant at least 18 years of age and have an acquired brain injury, including traumatic brain injury, infection, anoxia, aneurysm, tumor and stroke * must have valid Ontario Health Insurance Plan (OHIP) card * must have free will commitment to rehabilitation and/or participation in Dale Brain Injury Services programs * serious medical issues must be stabilized
