Description (Service) Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Community-led health teams develop programs to improve the social supports and conditions that affect long-term health

Primary Health Care Services - offered to rostered patients by a health care team of doctors, nurse practitioners, registered practical nurses, dietitian, social worker, chiropodist, helath promoter, physiotherapist and systems navigator including:
  • Foot Care - Chiropodist (foot care specialist) assesses and treats common foot problems * includes basic foot care and foot care for clients with diabetes
  • Mental Health Counselling and Social Work services
  • Quit Smoking - One-one counselling sessions
  • Mobile Unit - foot care visits and primary care visits to Aylmer 3x per month (Thursdays) for Mennonite clients
  • Transportation to specialist appointments at no cost * by referral only
  • Identification Clinic - Assistance with form-filling to replace lost or damaged identification cards such as birth certificates, Social Insurance Numbers (SIN) and health cards *for rostered clients

Health Promotion, Wellness and Community Programs - many programs offered to community members who do not need to be rostered patients of the Health Centre to receive services
