Description (Service) Help for people living with chronic conditions to make health behaviour changes and manage their illness * training is also offered to health service providers, friends and partners

People Living with Lifelong Conditions:

Living a Healthy Life with a Chronic Condition - Free 6-week program that helps people who live with an ongoing condition such as diabetes, arthritis, lung disease, etc. or experience issues such as chronic pain, fatigue, depression, anxiety or stress * available in communities across the South West

Living Healthy with Exercise - Free 8-week exercise program designed for adults who want to start becoming more active or maintain their balance, strength and flexibility * participants are led through 12 exercises and weekly action plans

Getting the Most out of Your Healthcare Appointment - Free 1-hour workshop with an action plan on how to prepare for appointments and what questions to ask

Health Care Professionals:

Choices and Changes Workshop - Provides clinicians with an opportunity to explore their own beliefs about the change process * specific strategies that can be used within the constraints of a typical office visit * uses videotaped case studies, live case situations and feedback from peers * accredited course through the College of Family Physicians of Canada
