Description and Service Details
Description (Service) Community support workers provide ongoing, individualized, 1-on-1 support to persons with serious and persistent mental illness 
 
* support provided through a continuum of case management functions including assessment, linkage to resources, person-centred planning; counselling, skill development, symptom management, advocacy, family support and public education 
 
* provides support and education * service is available throughout Essex County with satellite offices in Leamington and Essex
Hours Mon-Fri 8:30 am-4:30 pm
After hours and weekends by appointment only
