Description and Service Details
Description (Service) Community access service: 
* information and referral assists families with information, support and linkages to appropriate community services  
* Service Coordinators provide support while accessing relevant community agencies for children with developmental delays and/or mental health issues  
* facilitates access to services outside the community for families who have exhausted all community resources 
Outcome Based Program Information 
* Offer bridging services amongst service providers: assist through community case conferencing 
* Privacy and client confidentiality protcols: Require signed releases of information to share client information 
* Qualifications of service providers: BSW, MSW, RSW 
* Number of staff delivering programs: 2.5 
* Protocols for referral amongst agencies
Hours Mon-Thu: 9 am-5 pm; Fri 8:30 am-4:30 pm
