Description (Service) Primary Care Services 
* Weekly visits from physician(s) and on-call services at all times 
* Registered nursing care 24 hours a day 
* Diagnostic imaging, lab, and pharmacy services 
* Infection prevention and control program 
* Social work services including supportive counseling, discussion groups, advocacy, and financial management support 
* On-site laundry services 
* Daily housekeeping services 
* Physiotherapy 
* Occupational Therapy 
* Nursing Restorative Therapy 
 
In-Home Clinics 
* Dental/Dentures 
* Dermatology 
* Audiology 
* Optometry 
* Advanced Foot Care 
* Psychiatry 
* Mobility and Seating Assessments 
* Mobile Clothing Store 
 
Respite Care Program 
* Provides the caregiver with a break from caregiving duties 
 
Convalescent Care Program 
* Helps the resident to regain strength and confidence, usually following a stay in a hospital
Hours Mon-Sun 24 Hrs
Areas Served Essex County ; Windsor
Eligibility Seniors and older adults approved by LHIN, Home and Community Care Program for Long Term Care
Application * Application process required 
* The Local Health Integration Network, Home and Community Program creates and manages all applications for every long term care home in Ontario.  
* A phone call to the Home and Community Care Program is all that is needed to initiate an application to move into Berkshire Care Centre 
* A caseworker will schedule a time to come and visit the family or individual in the home or in the hospital.  
* The case worker will take care of completing an application package and send it to the Berkshire Care Centre
